This decreases the amount of red blood cell membrane and causes the formation of spherocytes (because of the low surface-tovolume ratio) muscle relaxant options order 500 mg methocarbamol. Spherocytes in a peripheral blood smear show a smaller diameter than normal and an apparent increase in hemoglobin concentration because of a decrease in cell surface spasms after hemorrhoidectomy 500 mg methocarbamol amex, with consequent deeper staining for hemoglobin muscle relaxant commercial order methocarbamol 500 mg without prescription. The osmotic fragility test (the shaded area in the graph reflects a normal response to spasms spinal cord injury generic methocarbamol 500mg fast delivery a hypotonic solution) can be used to document the presence of spherocytes in the peripheral blood. Spherocytes lyse at a higher concentration of sodium chloride than do normal red cells. In contrast, flat hypochromic cells, such as those in thalassemia, have a greater capacity to expand in dilute salt solution and thus lyse at a lower concentration (which is seen in the unbroken curve to the far right). The longer the incubation of the red cells in these salt concentrations, the greater the response to osmotic change. The differential diagnosis of spherocytosis in the peripheral blood includes hereditary spherocytosis and autoimmune hemolytic anemia. Macrophages within the spleen remove these bodies, producing characteristic "bite" cells. These red cells then become less deformable and are trapped and destroyed within the spleen (extravascular hemolysis). Two variants are the A type, which is found in 10% of African Americans and is characterized by milder hemolysis of younger red cells, and the Mediterranean type, which is characterized by a more severe hemolysis of red cells of all ages. Complement is activated by acidosis, such as with exercise or sleep, which can produce red morning urine. Microangiopathic hemolytic anemia refers to hemolysis of red cells caused by narrowing within the microvasculature and is seen in patients with prosthetic heart valves or those with disseminated intravascular coagulopathy, thrombotic thrombocytopenic purpura, or hemolytic-uremic syndrome. Because of the content of Hematology Answers 235 Hb A, erythrocytes only sickle at low oxygen tension and symptoms are much milder than in sickle cell disease. Hb F does not react with hemoglobin S, so that patients who have Hb S and large amounts of Hb F as occurs, in newborns with sickle cell disease, are asymptomatic. Clinically, sickle cell disease is characterized by the triad of chronic hemolytic anemia, vascular occlusion, and vulnerability to infection. The severe chronic hemolytic anemia leads to chronic hyperbilirubinemia (jaundice), which leads to pigmented gallstones. Vascular occlusion results in leg ulcers, renal papillary necrosis, and multiple infarcts, which may cause severe bone pain. Repeated splenic infarcts cause progressive fibrosis and splenic atrophy (autoinfarction). Rather than hypersplenism, most adults have a small, functionless, fibrotic spleen. The lack of splenic function along with defects in the alternate complement pathway predispose to infections such as Salmonella osteomyelitis and pneumococcal infections. The vasoocclusive disease also leads to painful crises, hand-foot syndrome in children (consisting of the typical triad of fever, pallor, and symmetric swelling of hands and feet), and infarctive crises. In patients not having yet undergone splenic autoinfarction (usually children), massive splenic sequestration (sequestration crisis) may lead to rapid splenic enlargement, hypovolemia, and shock. Patients with sickle cell disease have a normocytic anemia with a corrected reticulocyte count of greater than 3%. Hydroxyurea has recently been approved for the treatment of sickle cell disease because it increases the synthesis of hemoglobin F thus reducing the, severity of the disease. The test is based on the fact that erythrocytes with a large proportion of hemoglobin S sickle in solutions of low oxygen content. Spherocytes lyse at a higher concentration of salt than do normal cells, thus causing an increased osmotic fragility. The Schilling test, which measures intestinal absorption of vitamin B12 with and without intrinsic factor, is used to diagnose decreased vitamin B12 caused by pernicious anemia, which is characterized by a lack of intrinsic factor. Therefore, thalassemias are associated with a relative excess production of non-globin chains, while thalassemias are associated with a relative excess production of -globin chains. Most of the thalassemias result from deletions of one or more of the total of four -globin genes, while thalassemias result from point mutations involving the -globin gene. In contrast, deletion of only one gene on a chromosome (- /) is called thal 2 and is found in Africans. There are two possibilities for deletion of two genes: the deletions may be on the same chromosome (- -/, which is called the cis type) or the deletions may be on different chromosomes (-/-, which is called the trans type). The former, which is also called heterozygous thal 1, is more common in Asians, while the latter, which is also called thal 2, is more common in Africans. Clinically this is quite important because the off- Hematology Answers 237 spring of parents with the trans deletions cannot develop H disease or hydrops. This name results from the fact that excess chains postnatally form aggregates of tetramers, which are called hemoglobin H. The most severe form of thalassemia, hydrops fetalis, results from deletion of all four alpha genes (- -/- -). In patients with thalassemia, a deficiency of -globins causes a deficiency of hemoglobins that have -globin chains, and at the same time there is an increase in hemoglobins that do not have -globin chains (due to the excess chains present). Because of the need for repeated transfusions, over time these patients develop iron accumulation that leads to the formation of hemochromatosis. Indeed, congestive heart failure due to iron deposition within the heart is the major cause of death. Individuals with thal major have increased reticulocytes, increased hemoglobin A2, and markedly increased hemoglobin F (90%).

Vesicoureteric reflux refers to an incompetent valve between bladder and ureter, allowing reflux of urine up the ureter during bladder contraction and voiding. Relapse implies failure to eradicate the organism usually in association with anatomical renal tract abnormality. Neither urine dipstick testing for leukocyte esterase nor urine culture enhances diagnostic sensitivity. Most Gram-negative organisms reduce nitrates to nitrites and produce a red colour in the reagent square. Dipsticks that detect significant pyuria depend on the release of esterases from leucocytes. Dipstick tests positive for both nitrite and leucocyte esterase are predictive of acute infection (sensitivity of 75% and specificity of 82%). In patients with an indwelling urinary catheter, antibiotic treatment is indicated only in the presence of symptoms, and should be accompanied by replacement of the catheter. Most patients who delay antibiotic treatment to encourage spontaneous resolution eventually receive antibiotics and have longer times to resolution. Recurrent infection In patients with a relapse of infection, a search should be made for an underlying cause, with treatment of the cause if possible. In addition, intensive (1 week intravenous) or prolonged (6 weeks oral) antibiotics are required. Reinfection where there is usually no underlying renal tract abnormality is managed initially with lifestyle advice (2 L daily fluid intake), voiding before bedtime and after intercourse, and avoidance of spermicidal jellies and constipation. Colonization of the bladder by a pathogen is common after a urinary catheter has been in situ for more than a few days, partly due to organisms forming biofilms. So long as the bladder catheter is in place, antibiotics are likely to be ineffective and will encourage the development of resistant organisms. Only treat if the patient has symptoms or evidence of infection, and replace the catheter. Infection by Candida is a frequent complication of prolonged bladder catheterization. Treatment should be reserved for patients with evidence of invasive infection or those who are immunosuppressed. Early detection of asymptomatic bacteriuria and antibiotic treatment is necessary. It may be associated with vaginitis in post-menopausal women, irritant chemicals. Bacterial prostatitis Bacterial prostatitis is a relapsing infection which presents as perineal pain, recurrent epididymo-orchitis and prostatic tenderness, with pus in expressed prostatic secretion. Prostadynia (prostatic pain in the absence of active infection) may persist long after the infection. It is a feature of bacterial pyelonephritis but in this context it rarely, if ever, leads to renal damage in the absence of reflux, obstruction or other complicating factors. Renal biopsy shows an intense interstitial cellular infiltrate and variable tubular necrosis. High-dose prednisolone therapy is often used, although its value has not been proven. The prognosis is generally good; patients should avoid further exposure to the offending drug. Polyuria and nocturia are the result of tubular damage in the medullary area of the kidney, leading to defects in the renal concentrating ability. Necrosis of the papillae, which may subsequently slough off and be passed in the urine, sometimes causes ureteric colic or acute ureteral obstruction. In cases of analgesic nephropathy the drug should be stopped and replaced if necessary with paracetamol or dihydrocodeine. Investigations should be performed on all patients, although renal imaging is usually unnecessary. Essential hypertension Hypertension leads to characteristic histological changes in the renal vessels and intrarenal vasculature over time. These include intimal thickening with reduplication of the elastic lamina, reduction in kidney size and an increase in the proportion of sclerotic glomeruli. The changes are usually accompanied by some deterioration in renal function, which is much more common in black Africans. Accelerated or malignant-phase hypertension is marked by the development of fibrinoid necrosis in afferent glomerular arterioles and fibrin deposition in arteriolar walls. A rapid rise in blood pressure may trigger these arteriolar lesions, and a vicious circle is then established whereby fibrin deposition leads to renal damage, increased renin release and a further increase in blood pressure. There is progressive uraemia and, if untreated, fewer than 10% of patients survive 10 years.

A characteristic lesion muscle relaxant zolpidem discount methocarbamol 500 mg overnight delivery, called the florid duct lesion, is seen in portal areas and is composed of a marked lymphocytic infiltrate and occasional granulomas. The latter can result in the formation of pyogenic liver abscesses, which clinically cause high fever, right upper quadrant abdominal pain, and hepatomegaly. Infection with the ova of Echinococcus granulosus may produce a hydatid cyst within the liver, which is characterized by a thick, acellular, laminated eosinophilic wall (seen on x-ray as a calcified wall). The fluid within the cyst is granular and contains numerous small larval capsules with scoleces, called "brood capsules. Acute disease results in granulomas, while chronic infection produces a characteristic "pipe stem" fibrosis. Oriental cholangiohepatitis, seen in eastern Asia, is characterized by infection of bile ducts with Clonorchis sinensis. Benign tumors of the liver include hemangiomas (the most com- Gastrointestinal System Answers 347 mon), focal nodular hyperplasias, nodular regenerative hyperplasias, and adenomas. Hemangiomas are characterized by numerous small endotheliallined spaces filled with blood. The lack of erythrocytes or blood would raise the possibility of the lesion being a lymphangioma, while pleomorphic or atypical endothelial cells would suggest the possibility of an angiosarcoma. Focal nodular hyperplasia, which has a characteristic gross appearance of a central stellate scar within the tumor, microscopically reveals hepatic nodules surrounded by fibrous bands having numerous proliferating bile ducts. This type of tumor is related to birth-control pills, but has no association with malignancy. In contrast, nodular regenerative hyperplasia involves the entire liver and forms multiple spherical nodules. Histologic sections reveal plump hepatocytes surrounded by rims of atrophic cells. Nodular regenerative hyperplasia is clinically important because it is associated with the subsequent development of portal hypertension. Two types of hepatic adenomas are the liver cell adenoma and the bile duct adenoma. Histologically, cords of hepatocytes are present, but there is no lobular architecture. These tumors are associated with certain viral infections (hepatitis B and hepatitis C viruses), aflatoxin (produced by Aspergillus flavus), and cirrhosis. Microscopic sections of these tumors reveal pleomorphic tumor cells that form trabecular patterns, which are similar to the normal architecture of the liver. Clinically, hepatocellular carcinomas have a tendency to grow into the portal vein or the inferior vena cava and may be associated with several types of paraneoplastic syndromes, such as polycythemia, hypoglycemia, and hypercalcemia. It is important to compare the characteristics of hepatocellular carcinomas with those of another type of primary tumor of the liver, namely cholangiocarcinoma, which is a malignancy of bile ducts. Histologically, the tumor 348 Pathology cells contain cytoplasmic mucin, which is not found in hepatomas. Grossly there may be multiple or single nodules, which microscopically usually resemble the primary tumor. For example, metastatic colon cancer to the liver histologically reveals adenocarcinoma. Metastatic disease to the liver usually does not cause functional abnormalities of the liver itself, and the liver enzymes and bilirubin levels in the blood are usually normal. Angiosarcomas are highly aggressive malignant tumors that arise from the endothelial cells of the sinusoids of the liver. Their development is associated with certain chemicals, such as vinyl chloride, arsenic, and Thorotrast. Microscopically, these tumors consist of ribbons and rosettes of fetal embryonal cells. Cholesterol stones are pale yellow, hard, round, radiographically translucent stones that are most often multiple. Their formation is related to multiple factors including female sex hormones (such as with oral contraceptives), obesity, rapid weight reduction, and hyperlipidemic states. Decreased functioning of this enzyme, such as with a congenital deficiency or inhibition by clofibrate, causes excess secretion of cholesterol and an increased incidence of cholesterol gallstones. The other main type of gallstones are pigment stones, which are brown or black in color and composed of bilirubin calcium salts. They are found more commonly in Asian populations and are related to chronic hemolytic states, diseases of the small intestines, and bacterial infections of the biliary tree. In contrast to 7-hydroxylase and bile acid synthesis, 1-hydroxylase is involved in vitamin D synthesis, while 11-hydroxylase, 17-hydroxylase, and 21hydroxylase are all enzymes found in the adrenal cortex. Jaundice secondary to extrahepatic obstruction is associated with normal hemoglobin levels, normal serum indirect bilirubin levels, and increased levels of direct bilirubin and alkaline phosphatase. Common causes of obstruction of the common bile duct include cancer in the head of the pancreas and obstruction by a gallstone. In contrast, most patients with gallstones have cholecystitis, which is associated with a thickened gallbladder wall that prevents the gallbladder from dilating. Therefore, if the obstruction is due to a gallstone, the gallbladder will not dilate and will not be palpable. Cholecystitis (inflammation of the gallbladder) may be either an acute or a chronic response. In acute cholecystitis, which may be associated with a stone (calculous type) or lack a stone (acalculous type), there is an acute inflammatory response that consists mainly of neutrophils.

Be aware that a sudden loss of intrauterine pressure may indicate perforation, resulting in shock. The circulator should be certain that the drug is the correct one, at the correct strength. The scrub person should report the breakage and loss if the object is not found; the loss must be reported to the surgeon and the circulator. Surgery for Ectopic Pregnancy Definition Surgical treatment of a pregnancy established at a site other than within the uterine cavity. Discussion the fertilized ovum may implant (rarely) within the uterine tube, ovary, peritoneal surface, broad ligament, and cervix. The fallopian tube is the most frequently involved other site, usually within the ampullary portion. Many small ectopic pregnancies abort and go undetected, with minimal abdominal pain the only symptom. In small presentations, in the medically stable patient, methotrexate (with leucovorin) can be given systemically, or methotrexate, potassium Chapter 18 Gynecologic and Obstetric Surgery 327 chloride, prostaglandin fraction, or hypertonic glucose can be injected directly into the fallopian tube (salpingocentesis) under laparoscopic or transvaginal ultrasound guidance. Laparoscopic treatment is preferred when there are no contraindications, although simultaneous fine-suture tuboplasty to preserve fertility is not often successful. When feasible, the gestational tissues can be expressed from the fimbriated end of the tube, but this is not usually recommended unless spontaneous abortion has begun. Otherwise, the tube can be unduly traumatized or trophoblastic tissues will not be completely removed. Fallopian tubal rupture with hemorrhage requires laparotomy, with fine suture repair of the tube or primary tubal anastomosis after partial tubal resection. Preservation of fertility may be dependent upon status of the contralateral tube, size of the pregnancy, previous history of ectopic pregnancy, and the availability of in vitro fertilization to the patient. Indications for salpingectomy include hemorrhage, tubal rupture, lack of desire for further pregnancy, and history of prior ectopic pregnancies. Pneumoperitoneum is achieved and two or three laparoscopic ports are established; see Gynecologic Laparoscopy/ Pelviscopy, p. If the pregnancy occurs in the ampullary portion and the tube is to be preserved, a linear incision is made and the gestational tissues are removed. Hemostasis is achieved; the tube may be left open (to heal by second intention) or fibrin glue may be applied, or fine sutures may be placed. For salpingectomy, a dilute solution of vasopressin (4 u/20 ml saline) may be injected into the intended line of incision (to reduce bleeding). The mesosalpinx is divided, ligating larger vessels, and the tube is excised at the isthmus by Endoloop ligation. Alternatively, a segment of traumatized tube can be resected with plastic repair secondarily. Suturing a portion of broad ligament or round ligament to the tubal stump is thought to minimize adhesions. Laparotomy affords the ability to rapidly aspirate blood, detect and control bleeding, and perform salpingectomy. A lower abdominal incision is made; the operative field is cleared of tissue debris, and the bleeding is controlled. Tubal preservation can be attempted with fine-suture (6-0, 7-0) repair of the tube, or if a traumatized section is excised, direct reanastomosis can be performed if the tissues are deemed salvageable. Otherwise, resection and secondary repair of the tube can be 328 Chapter 18 Gynecologic and Obstetric Surgery done. For salpingectomy, the procedure is similar to that described above, using ordinary surgical techniques. In the rare presentation of interstitial ectopic pregnancy, especially if rupture has occurred, wedge resection of the involved area or even hysterectomy (see p. When the uterus has been operated upon, Cesarian section for subsequent pregnancy may be advised. Preparation of the Patient Antiembolitic hose may be put on the legs when requested. Following the administration of general anesthesia, the patient is placed in a modified lithotomy position using padded stirrups. The table is placed in Trendelenburg position; this position encourages organs in the peritoneal cavity to fall cephalad, permitting a better view of the operative area. The perineum and anus are prepared last; discard each sponge after wiping the anus. For the abdominal prep, begin at the intended site of incision (usually Pfannenstiel), extending from nipples to mid-thighs and down to the table at the sides. Drape sheet under the buttocks, followed by a laparoscopy sheet, or drape sheet, leggings, and a laparotomy sheet Laparotomy Approach. There must be two working suctions (in addition to the one on the anesthesia cart) in the room in case hemorrhage should occur. Keep an accurate record of irrigation fluid used to assist in determining blood loss and fluid replacement. Tuboplasty of the Fallopian Tubes Definition the reestablishment of patency to the fallopian tubes. Discussion Tuboplasty is usually performed when the patient seeks a reversal of a sterilization procedure.
